To book a consultation as a new patient:
Option 1:
Have your doctor (family doctor, walk-in doctor, specialist) fax a referral at 1-833-457-1690. We will contact you to book an appointment.
You can download our referral form here and give it to your MD. Once we receive the referral, we will contact you for an appointment. In the meantime, you can fill your history form here:
